As an alternative medicine practitioner i can tell you for a fact that what you have is LUPUS.

It is an autoimmune disease in which the body's immune system becomes hyperactive and attacks normal, healthy tissue. Causing inflammation and damage to the joints, skin etc.

Skin infections or problems might not be life threatening but the can truly affect the quality of life.
The most important factor in treating an infection is a diagnosis. Go to a dermatologist, after consultations you might take skin snips and swab and cultured to find out the the whole aetiology of the condition. Then if its is a fungal infection which I suspect it is, then you'd be placed on antifungal, the route depending on the severity.
For now, get a terbinafine brand of antifungal cream and you can use Oral Nystatin or Griseofulvin. If you use for 14 days and no changes go to a dermatologist..

In 2013 when I was training to join the Army, I developed this rash on my back just where you have yours. It started small and before you know it, it spread a to almost all of my back.
I did all I could but it didn't work till I used SKINEAL religiously for about a month and it finally disappeared.
Then it would become very itchy if there's heat and I swear on my back. But now it's gone. So I suggest you try it too. Good luck.
