Tight braids for black men and women are often associated with Traction Alopecia hair loss. But what most people don’t quite realize is that the intense pulling caused by braiding hair too tightly may contribute to Acne Keloidalis Nuchae (AKN) a condition which has been shown to affect ethnic men who are genetically predisposed to this progressive skin malady.

Constantly pulling the hair creates permanent damage to the follicles. The skin (of affected individuals) then tries to heal these tears and wounds by constantly producing collagen. This causes what starts off as tiny, small bumps to keep growing. The process often continues until the tissue lesions get even larger and merge together into one large mass.

The onsetting trigger for this patient’s Acne Keloidalis Nuchae was braiding his hair too tightly. Thinking that the initial small lesions were just common razor bumps that would go away, he held off on seeking treatment. The bumps kept growing for 19 years until they became a huge lesional mass on the back of his scalp.

Nigerian native, Dr.U (Sanusi Umar MD), excised the AKN which left a big, gaping wound on the back of this patient’s head. With a greatly reduced skin surface area remaining on the scalp, it became necessary to then gradually close the wound over time using tension sutures.This pulled the edges of the wound together, while allowing the skin to grow new tissue until it resulted in a final, cosmetically acceptable linear scar.
