5.Original and photocopy of duly completed medical form (To be done at school with #7,500 and x-ray of 1,000 anywhere you like).

6.Letter of attestation of good behaviour.

8.Parent valid tax clearance.

9.Evidence of payment of #500 Osun state capital project development levy (To be paid at tax offices in the State of Osun).

The best is to come down to Osun state and do everything as acceptance at Lasgidi without submission at your college office equivalent to Zero point...............
