Ibime: CT will usually pick up the obvious bleeds and whatnot and also almost all the tiny ones too. However if a patient is still not himself after a couple of weeks and the doctor does not believe it is just concussion (which is the most likely explanation) next step would be an MRI which is a higher level of imaging. To reassure you a bit. Seizure which occurs immediately after the head injury and lasts only for a few minutes followed by return to normal conscious state is almost always benign compared to the ones that happen hours to days after the injury or last for several minutes or when there is no return to normal or when the seizures are recurrent. If the latter happens that is when alarm bells start ringing. 3 Likes |

Ibime: It could well be. That is why i said your boy must have barely qualified for the CT in the first place. Some other units might have held off on the CT and just observed for a longer period especially when the history of true seizure is not clear cut. We try to avoid CT in children if possible because of the radiation. In some cases though it has to be done. |

donjazet: You see why I can't easily denigrate this country. Before the ambulance came, a solitary paramedic arrived by car (first responder who can get there quicker than the main ambulance) then the ambulance came, then another senior medic came in a different car came shortly after (who must be a doctor sent to support paramedics in life threatening situations). Before they came, 999 operator has already coached us through performing CPR. Before ambulance reach Hospital, they have already organised Consultant to be on standby with crash team. From entering the hospital to CT scan took 10 mins and in another 5 mins after that, X-Ray was ready for cervical scan. Nobody asked my skin colour or whether I pay taxes or have insurance before treatment. The lead Radiographer on the CT was one black boy (looks in his mid-20s) whilst the lead Radiographer on the X-Ray was one black girl (can't be up to 30). The nurse-in-charge in step-down observation unit after he had recovered sufficiently was one Somalian girl also mid-20s with hijab on her head. This country is trying abeg. afrodoc2: This is a specialist children's hospital (MRI Manchester). Even the CT machine looked smaller than what I'm used to. |

afrodoc2: I would imagine a CT was ordered because the story states he hit his head. @Ibime correct me if I am wrong, they should have asked the length from where he fell, trying to determine the impact level. Before ordering the CT, the doctor would have rubbed his hand across Ibime’s son’s head looking to feel trauma or facial changes/ reaction to pain. I think he was trying to play safe based off the story and you are right about radiation, you want to avoid exposure. |

Roland17: Yeah exactly. Sometimes you get a physician who is very cautious one way and other times a physician who is cautious the other way. And you are absolutely right the mechanism of injury of which the height of the fall is an important aspect is very crucial. Ibime described a fall from a low height which is another reassuring factor BUT what was the surface that the head hit is another factor. Examination is also key and there are several things to look for. One thing experience has taught me is that once the initial response is high level escalation with ambulance and lights n sirens it is often difficult for anyone to have the courage to now say “no need for CT”. Rather they would play safe and do the CT so everyone is happy and the parent won’t sue them later for negligence and not investigating properly. |

BTW, I've always acknowledged that Western Europe have improved tremendously in race relations at least compared to the United States but you have to agree with me that it's not yet perfect, there are still elements of the old guard scattered across the country. The disproportionate incarceration rate is a pointer to this. The presence of proud white supremacists group is another. But on the whole, I do agree that there has been a great improvement in race relations than was seen few decades ago. Ibime: We already have numerous midfielders, do we really need more? I just want to us take care of our goal keeping problem.. Kepa will not change overnight. He lacks good communication with his defense, he has very poor ariel command, he cannot come out to grab the ball or punch it well away when we concede set pieces. He loses focus easily, he has weak hands for a keeper and many more problems. This area has been my chief area of concern, not the midfield that is fully stacked. |

donjazet: Kepa will be dealt with surely, I just think VDB is an upgrade on Barkley, Kova and Mount. I won't say he's better than RLC attacking wise but he is miles above RLC in controlling midfield. |
