I need clarification under "PERSONAL HISTORY"

I served with company X for NYSC and I was offered full employment after service. This was after six months.

Now, under "WORK HISTORY" I only filled the period I became permanent staff which is what I'm claiming points for.

Hope it is not a problem if I state NYSC & the same EMPLOYER under "Personal History" and I make no mention of it under "Work History"?

I created a 2nd Express Entry profile as my 1st was ineligible, and in the new profile, when they asked "Have you applied for Express Entry before", do I answer yes and entered the EE profile number of my previous ineligible profile or no.
