It is with a heavy heart I write. I can't contain it anymore. What started some three years ago is about to send a young boy who has barely started life to the great beyond. His name is ONYEJI ANTHONY MMADUABUCHI. He is my brother, the 5th child in my family. He hals from Abajah, Nwangele LGA, Imo State. He had nephrotic syndrome three years ago and was vigorously treated. Complete remission was achieved. The nephrotic syndrome struct again last year and progressed rapidly to End Stage Renal Disease.

He has been off and on dialysis since January this year and the laboratory results are not getting favorable. He was admitted in National Hospital Abuja for nearly a month. He has been discharged and is currently going for dialysis in FMC OWERRI. There is still the need to move him to INDIA where he can get kidney transplantation.

Since you say he is in an abuja hospital, I suggest that you look for Abuja based nairalanders who can visit him and help to verify

My brother, I don't know of any nairalander in Abuja. But I have given his location in the hospital with recent receipts of payment. Anybody can call the hospital to confirm the news. Brief Introduction
Malaria is a disease caused by malaria parasites. There four known Plasmodium parasites: P. falciparum, P. vivax, P. malariae and P. ovale. Plasmodium falciparum causes the deadliest form of malaria. The parasite is transmitted by female anopheles mosquitoes. The prevalence of malaria in Nigeria is very high and the country is said to be a holoendemic zone. The reason is because malaria affects Nigerians throughout the year. There is no break in transmission and it does not follow seasonal changes.

On the other hand, typhoid fever, otherwise known as enteric fever is caused by Salmonella typhi and Salmonella paratyphi. Typhoid fever follows a faeco-oral transmission. What this means is that you can get the disease if you eat food or drink beverages handled by someone that is shedding the bacteria. Also, in Nigeria, like in other developing countries, where defecation in open places is common, transmission can also be water-borne. In this instance, some villagers drink water from streams without purifying it. And by so doing, get infected with typhoid.

Clinical Features
The clinical manifestations of malaria include fever, chills and rigor, headache, malaise, joint pains, nausea and vomiting. Others are diarrhoea, convulsion (especially in young children) and coma. Malaria is a serious disease and can cause complications like anaemia, liver failure and jaundice, dehydration, low blood sugar, acute respiratory distress syndrome, swelling and rupturing of the spleen, kidney failure and pulmonary oedema.

Typhoid on its own can cause fever, colicky abdominal pain (perforation of the small intestine can occur), joint pains, diarrhoea, cough ,nausea and vomiting headache, muscle pain, confusion and can mimic some of the symptoms of malaria.


Diagnosis
Diagnosis of malaria involves various approaches which include using microscope to observe the presence of the parasite in blood, rapid diagnostic test (RDT), serological test to determine presence of antibodies against the parasite, and more importantly, a high degree of suspicion. This is of particular importance in a resource-poor setting where a microscope may not be readily available. In this case, the interpretation of the RDT must be done with good clinical acumen because as false positive and false negative results can be obtained...

Nowadays, people aspire for an increased standard of living which they attribute to the fatness of their bank accounts, the number of cars in their fleet, the massiveness of their mansions, the expensiveness of their meals, clothing and jewelry, etc. Everybody wants an easier version of everything and tends to be in a hurry. The life we are living is fast-paced; faster than our forefathers lived. The infotainment superhighway is not slowing down any bit. If anything, it is spiralling out of control. We are so engrossed in making dough that, sometimes, we forget to take care of ourselves. We spend a good spell of our lifetime focused on making money, not bothering to take out some time to evaluate how well we are living. And at the end, many will spend a lot of money managing preventable terminal diseases and dying horrible deaths. Does it worth it? Yes, we have to make money. but that should not come at the expense of our health because health itself is the greatest of all wealth.

The WHO made it clear to all and sundry that health is not just the absence of diseases, but a total physical, mental and social well-being of every individual. It is an inalienable right of everyone. Of course, you have the right to good health. Extrapolating from the WHO definition, that optimum health remains elusive. And I do not think that there is anyone out there without a health issue, not just the physical well-being, one has to take into consideration the social and psychological dimensions, especially as we are weighed down by both economic austerity and terrorism.

Hence, one question I would like to ask everyone (myself inclusive) is when last did you measure your blood pressure? How much do you weigh? And what is your height? It sounds stupid and funny, right? Well, let me bring it to your knowledge that a two-digit percent reduction in mortality will be achieved if these parameters are well-controlled. I am confusing you the more in this discourse! You are damn right. Let us take it slow. Do you know that sustained abnormally elevated blood pressure and body mass index (BMI) above 30 are the hallmarks of hypertension, diabetes, dyslipidaemia, etc Together, these diseases cause more than a quarter of the mortality recorded worldwide.

Hypertension is defined as systolic blood pressure of 140 mm Hg or more, and/or a diastolic blood pressure of 90 mm Hg or more. Hypertension affects a lot of people worldwide. It is a major risk factor for stroke, myocardial infarction, vascular disease and chronic kidney disease. In some 95% of the population, the cause of hypertension is not clearly understood. By this I do not mean that it has spiritual cause as some of us have been deluded to conclude. What I mean is that the cause is polygenic and multifactorial. Both hereditary and environmental factors come into play. Hence, it runs in families. The environmental factors can be cultural or occupational etc. The remaining 5% are caused by other disease conditions. The pathology involved is outside the scope of this discourse. On the other hand, obesity can be caused by over-indulgence in food and drinks (alcoholic beverages), metabolic disorders like diabetes mellitus, hypercholestrolaemia, enzyme deficiencies and sedentary lifestyle.

The Misconception

For those who have been diagnosed earlier and are currently living with hypertension, regular check of blood pressure cannot be over-emphasized. I see people who tell stories of how their hypertension was cured miraculously. Yes, I believe in miracle, but I would like to have a word with you. Let me explain to you in layman's term how you might have mistaken a pathological process for a miracle. When, for whatever reasons, the heart is no longer able to supply the rest of the body and itself of adequate blood, a cascade of mechanisms set in and ensure that the metabolic demand is met...

An Overview


Ultrasound scan is a safe and painless procedure of producing images of inside the body using sound waves. It involves the use of a small transducer (probe) and ultrasound gel placed directly on the skin. High-frequency sound waves are transmitted from the probe through the gel into the body. The transducer collects the sounds that bounce back and a computer then uses those sound waves to create an image. Ultrasound scan does not use radiation as opposed to X-rays, CT scans and MRI. It is also non-invasive. That means there is no need to do any form of open surgery in order to have a better visualization. It captures motion pictures in real-time, and hence, can be used to visualize the inside of the body and blood flow through vessels. Conventional ultrasounds displays its images in 2D. However, recent advancements have led to the production of 3D ultrasound that may incorporate Doppler. The latter helps in visualizing blood flow.

The ultrasound has various components: there is a console containing computer and electronics, a transducer or probe connected to the console through a cord and a visual display screen. The console is like the central processing unit of a computer, the probe is like a mouse and the visual display unit is the monitor; although they do not perform similar functions. These are the basic components. However, portable ultrasound machines can have console and monitor compressed into one.

Indications for Ultrasound

The ultrasound scan can be used for the heart and blood vessels, liver, spleen, kidneys, bladder, uterus, ovaries, unborn foetus, scrotum and thyroid. This list is, however, not exhaustive. Usually, the patient to be examined will be positioned lying face up. The examining table can be ti
lted to get a better exposure. The area to be examined is uncovered and a warm inert (non irritant) gel is applied on it. The transducer is subsequently maneuvered over it to visualize the structures underneath it. the gel is wiped off at the end of the examination. The probe can also be passed into the esophagus (in transesophageal echocardiogram), transrectally (to view the prostate) and transvaginally (to view the uterus).
