The Nigerian Safety Investigation Bureau (NSIB) has launched an investigation into a serious incident involving a Fly Bird HS 125 aircraft, registration number 5NKAL.

The aircraft, which was en route from Nnamdi Azikiwe International Airport, Abuja (DNAA), to Kotoka International Airport, Accra (DGAA), experienced technical failure on December 6, 2024.

During the flight, the aircraft experienced a loss of both engines. Initial reports indicate that while climbing to flight level 280, the crew reported an issue with engine number 2.

Shortly after, the aircraft was approximately 25 nautical miles southwest of the VOR station VONUK when the crew requested to divert back to Abuja.

The request was immediately granted, and the aircraft descended to flight level 190. However, during this descent, engine number 2 failed at flight level 230.

As the situation worsened, the crew declared a Mayday emergency as engine number 1 also failed at approximately 5,000 feet while approaching Nnamdi Azikiwe International Airport.

Despite the challenges, the crew successfully landed the aircraft at Abuja Airport at 18:16 UTC, with all four people on board – three crew members and one passenger – unharmed. There were no injuries reported.

In a statement issued on Sunday, Mrs. Bimbo Olawumi Oladeji, Director of Public Affairs and Family Assistance at the NSIB, confirmed the incident and revealed that a full investigation is underway.

Preliminary findings reveal that the aircraft was climbing through flight level 240 when the engine issue was first reported.

It is good that no life was lost. But imagine what would have been the outcome if the flight was fully loaded.

Thank God. I hate flying but unfortunately have to endure it as a means of transport. Once i enter, i just zero my mind until i land again.

The state of maintenance as well as age of some of this planes are worrisome too.

Most times it's just God's mercy that preserves lives when aircrafts experience issues during flights.... Almost every day you must hear of one thing or the other during flights. Sincerely most of the aircrafts have tried, you'll see aircrafts of 40-50years old still in use, just keeps getting refurbished.

Glad the passengers and crew landed safely. Most likely reason is poor maintenance / faulty parts.

If fully loaded nothing will happen. Because the second engine failed right there in thr airport while descending..but if the second engine had failed when the plane was away from airport then it will be forced to land or crash at a spot.

Thats why plane is equipped with two engines. If one fails the other will still serve but why the two engines failed calls for concerns

The aircraft involved is probably small that's why it was able to glide to safety at the airport, if it were a much larger aircraft the story might have been different undecided

I once used an Azman aircraft from Abuja to Kano that had adhesive tapes on the wing I was staring at through my window seat.

Almost everything in this country is designed to unalive you.

It doesn’t just work like that, lost two engines at 5000feet? They must be very very close to the airport (few kilometers) away from the airport for them to have survived.

If not, there was no way that big plane would had survived on gliding at 5000feet without any functional engine.

Yes, they must have been within the aerodrome and most likely at short final. Again, managing the aircraft aerodynamically gave them an edge.

You can't be in Delta at 5,000 FT gliding to Lagos airport which is about 300km away. You must be closer to the airport to succeed at that.

May day means okwu Agwula... I doubt both engines actually failed because, landing successfully without crash landing is almost impossible.

This is why airplane is still the safest means of transport. Even if the engines are dead, you can still safely land the airplane.

Those are not the normal tapes you see around...what you saw was probably an aerospace tape used in the aviation industry but the non aviatic mind go think say na cellotape. grin

Unlike some pilots, this particular pilot's obedience to flight instruction saved the day. His quick thinking to return to the airport rather continue the flight on one engine saved the crew and passenger. Some would argue that one engine can still fly the plane, but why take such risk rather than just abort the flight all together.

Why do you think they made a decision to turn back? Cos they knew they could make it back to LAG rather than their destination aerodrome.
