South Africa has the biggest and most high profile HIV epidemic in the world, with an estimated 7 million people living with HIV in 2015. In the same year, there were 380,000 new infections while 180,000 South Africans died from AIDS-related illnesses.1

South Africa has the largest antiretroviral treatment (ART) programme globally and these efforts have been largely financed from its own domestic resources. The country now invests more than $1.5 billion annually to run its HIV and AIDS programmes.2

However, HIV prevalence remains high (19.2%) among the general population, although it varies markedly between regions.3 For example, HIV prevalence is almost 40% in Kwazulu Natal compared with 18% in Northern Cape and Western Cape.4

Key affected populations in South Africa

South Africa's National Strategic Plan 2012-2016 identifies a number of key affected populations that are at risk of HIV transmission.5

Men who have sex with men (MSM) and HIV in South Africa

HIV prevalence among men who have sex with men (sometimes referred to as MSM) in South Africa is now estimated at between 22% and 48%. HIV prevalence among men who have sex with men varies geographically but it is reported to have risen by more than 10% in Johannesburg, Cape Town and Durban.6

Many men who have sex with men still face high levels of social stigma and homophobic violence due to traditional and conservative attitudes. As a result, men who have sex with men find it difficult to disclose their sexuality to healthcare workers, limiting their access to HIV services.7

However, there is evidence that attitudes are changing. In 2013 a study found that only 32% of South Africans said homosexuality should be accepted by society.8 A more recent study in 2016 found 55% of South Africans would accept a gay family member; 51% said gay people should have the same human rights as others; and two thirds supported keeping the constitutional protections against discrimination on the basis of sexual orientation.9

However, the same study also found that 72% of people said same-sex sexual activity was morally wrong. 18% either had, or would consider, verbally abusing someone who is not gender conforming – and nearly 10% had, or would consider, physically abusing them.10

South Africa remains the only country in sub-Saharan Africa where gay rights are formally recognised.11 Moreover, national policies strongly emphasise equity, social justice and forbid discrimination based on sexual orientation. These are prerequisites for the provision of HIV services for men who have sex with men, as well as other members of the lesbian, gay, bisexual and transgender community. As a result, South Africa has the potential to provide a leading role in the improvement of HIV service provision for men who have sex with men throughout sub-Saharan Africa.12

Sex workers and HIV in South Africa

Nationally, HIV prevalence among sex workers is estimated at 59.6% although this varies between areas, with prevalence estimated at 72% in Johannesburg, 54% in Durban and 40% in Cape Town.13 14 In 2010, sex work accounted for an estimated 19.8% of all new HIV infections in South Africa.15

Female sex workers are particularly affected, with studies finding HIV prevalence among this group to range from 40% to 88%, significantly higher than among women in the general population (14.4%).16 17In particular, young female sex workers (under 25) carry a heavy HIV burden, highlighting the need for focused interventions for sex workers from an early age.18

Sex workers in South Africa face high levels of stigma and discrimination and are restricted by the laws under which they work. Moreover, many sex workers also inject drugs, exacerbating their vulnerability to HIV infection.19

Educational organisations have reported difficulties in delivering HIV prevention services to sex workers due to ongoing police harassment.

In 2010, only 88% of people in South Africa with advanced HIV/AIDS were receiving anti-retroviral treatment (ART). In 2004, 2005 and 2006 the figures were 4%, 15% and 21% respectively.[3] By 2009, nearly 1 million or about 2% of all adult South Africans were receiving ART.[4]

In 2010, an estimated 280,000 South Africans died from the effects of HIV/AIDS. In ten years preceding, it is estimated that between 42% and 47% of all deaths among South Africans were HIV/AIDS deaths.[5] However, the Death Notification Forms Survey of 2010, which estimates a 93% completion rate, shows that out of a total of 543,856 deaths nationwide (Appendix C4), only 18,325 deaths were attributed to HIV/AIDS Diseases (B20-B24, Table 4.5).[6]

Although new infections among mature age groups in South Africa remain high, new infections among teenagers seem to be on the decline. HIV/AIDS prevalence figures in the 15–19 year age group for 2005, 2006 and 2007 were 16%, 14% and 13% respectively.[7]

The Human Sciences Research Council, a South African institution, estimates 10.9% of all South Africans have HIV/AIDS.[8] Additionally, the Central Intelligence Agency estimates that 310,000 individuals died in South Africa from HIV/AIDS in the year 2009.[2] The rising prevalence rate has increased from 10.6% in 2008 to 12.2% in 2012. In 2012 alone, the Human Science Research Council (HSRC), reported 470,000 new diagnoses—or nearly 1,100 new infections every day. That's 100,000 more than was seen just one year earlier in 2011. Driving these statistics is the decreasing, rather than increasing, public knowledge about HIV. According to the HSRC report, only 26.8% of the 38,000 people surveyed understood how HIV was transmitted or ways to prevent it. That's down from 30.3% in 2008, with evidence showing that South Africans under 50 are having an increasing number of sexual partners and lower condom use.

HIV/AIDS is more prevalent among females, especially those under the age of 40. Women made up roughly 4 in every 5 people with HIV/AIDS aged 20–24, and 2 out of 3 of those aged 25–29.[citation needed] Although prevalence is higher among women in general, only 1 in every 6 HIV/AIDS infected people with multiple sex partners are women.[citation needed]

HIV prevalence among pregnant women is highest in the populous KwaZulu-Natal province (37%), and lowest in the Western Cape (13%), Northern Cape (16%) and Limpopo (18%) provinces. In the five other provinces (Eastern Cape, Free State, Gauteng, Mpumalanga and North West) at least 26% of women attending antenatal clinics in 2006 tested HIV-positive.

On a different note,I don't understand how the Ghanaians here can say what they say about Nigeria.We are trading partners;Nigeria is Ghana's 2nd largest importing partner after China.Besides I thought we were friends.

There are way more people who live in denial of the actual existence of the virus. Just look for HIV threads on this forum and you'll be shocked at how many Nigerians are in denial.

May be drugs are sold in plain sight in Naija, but in places where it's illegal, they are hidden in other products,
