thorrvik55: This one is the funniest and most intriguing. I know a couple that the bride family are rich and the groom family are just there. Initially the bride family was against both of them marrying but they later consented after persuasion from their daughter. Long story short, the bride family gave the couple 5m for the wedding and they spent all the 5m on the wedding without even saving 1kobo. 3 months later things hit, the bride family refused to support them with a kobo, they told them that whenever they are hungry, they should watch the video of their wedding again, they will be full. 13 Likes 2 Shares |

Ibime: Interesting read. I actually watched the Kyle-Sidell video a couple of weeks ago and some of what he said was eye opening, in particular seeing patients with severe hypoxemia who appear clinically stable. Critical care specialists and emergency physicians globally are still learning about this disease, which is why there is this ongoing debate about the optimal protocol for management. My guess from what i have read is that COVID-19 does not attack the lungs of everybody the same way. You have some whose lungs show evidence of oedema and reduced compliance and would benefit from invasive mechanical ventilation (ARDS/Pneumonia picture) , and then you have others whose hypoxia is not due to lung oedema or reduced compliance but due to a ventilation/perfusion mismatch in the terminal areas of the lungs where gas exchange takes place(High altitude sickness picture). The first group should benefit from ventilation but the second group might actually suffer from ventilation as the pressures needed in ventilation might cause damage to their lungs which were not damaged before. I am not sure what role CPAP plays because you have to remember that CPAP still involves applying pressure so that the theoretical damage that can be inflicted by ventilators can also be inflicted by CPAP as well. However CPAP is less efficient and therefore you are maybe causing structural lung damage without giving much oxygenation benefit. There is also the higher risk of aerosolization and transmission of infection with CPAP. In summary i think critical care physicians, emergency physicians, radiologists, and pathologists (carrying out autopsies) have to come together and discuss their findings before a consensus can be reached on how best to treat COVID-19 patients. 5 Likes 2 Shares |

afrodoc2: Understood. Do you think there might be some marginal benefit in keeping a CPAP at home for early stage? My brother in-law suffers with COPD and keeps a Bipap, nebuliser and such. From there, I learned about these things and bought a nebuliser and some saline to get my lungs in some shape for Covid. Had thoughts on a CPAP but dismissed it but this is making me have a rethink. |

Ibime: CPAP theoretically may offer some benefit in terms of how we understand COVID-19 to attack some patients. However how do you decide on the right settings to use in your specific case. High flow nasal oxygen and CPAP are all options but if someone really requires them then he/she should be in the hospital. |

odizeey: All Chief of Staff round the World are very powerful. Many times the President is just the final signatory to what COS has investigated and decided. Check Obama and Rahm Emmanuel. The weaker the President, the more falls on the COS. When you have a President with Health Issues, the COS starts making Executive decisions. |
