1)Since you are in Lagos,write the address of US embassy in lagos there.

2) Though yoruba language has native alphabet,i would advice you to write[b] N/A [/b]in that area. That was what i did during my own time and nothing happened.

3)write the address of the person you are going to stay with over there in US

I think the mistake can be backed up by your certificates(birth cert and school certs) cos correcting the mistake on your passport may take months and cost you alot. You may be required to visit Abuja several times before they can make any changes. I would advice you fill the form with the correct name you have on your certificate,write a small note telling KCC about the mistake and send it along with the forms.

weyinmibaby: Hello gurus, i was selected for the DV lottery and i just downloaded the "SUPPLEMENTAL REGISTRATION FOR THE DV IMMIGRANT VISA PROGRAM" Now the expiring date on the form is 01/31/2013 and i dont have ay problem with that, But the second form i downloaded and want to fill is the "APPLICATION FOR IMMIGRANT VISA AND ALLIEN REGISTRATION" FORM But the expiry date here is 02/29/2012. Please i need help, is this the same form you all filled? Where can i get a current one that has not expired?The place i got the forms is stated below in their website. Also please i would like to know the full address of the united states consulate office here in nigeria where the applications would be processed

Supplemental Registration for the Diversity Immigrant Visa Program, Form DSP-122: Only the principal applicant needs to complete Form DSP-122.
Application for Immigrant Visa and Alien Registration, Form DS-230, Part I and II: The principal applicant and all family members applying for a diversity visa must submit both Part I and II.
Note: Each Form DS-230 Part II, Sworn Statement should be submitted unsigned.
Two (2) photographs of yourself and of each family member applying for a diversity immigrant visa. Ensure that the photos comply with the General Photo Requirements.
