Comcerning the pics.... I think the short guy has achondroplasia... The tall one, i am not so sure, but it looks like pfeiffer's or cruzorn's syndrome... I cant see the pics clearly cos i'm using my c3 |

It's achondroplasia A genetic disorder of cartilage. The most common cause of disproportionate short stature(that is arms and legs are short in proportion to body length). Most cases are not inherited. The inherited ones occur in autosomal dominant pattern. They usually have short limbs, large head with prominent forehead, mis-aligned teeth,trident hand. Diagnosis can be made before birth by fetal ultra sound and DNA tests. And after birth, by medical history and physical examination. Complication include hydrocephalus,bony deformities(lordosis, kyphosis, bowed legs), inability to walk, spinal stenosis. There's no specific treatment or prevention. Complications are treated as they arise. The patient and the family may benefit from medical counselling. Options: *growth hormone therapy *Leg lengthing surgeries. It's important to note that aside all these problems they face,their intelligence is not affected. |

A 35yr old male was refered to you with two days history of deformity on the right thigh and inability to use the right leg following a Road traffic accident. Nil history of obvious bleeding or fluid discharge from craniofacial orifices. No loss of consciousness. Chest and abdomen,clinically clear. No obvious wound on the skin. He was stabilized in the hospital before the referral,thus the patient is essentially stable. The x-ray film shows: * Undisplaced fracture of neck of right femur * Communited fracture, mid shaft of the right femur. Patient is admitted. Placed on analgesics. Pre op investigations. Cross match at least 2-3 pints of blood. Considering the femoral neck, the initial management with traction (e.g skin traction) is somehow dicey, i guess it's open for debate. Definitive treatment: Fluoroscope is required. General or regional anesthesia can be used. *For femoral neck fracture- Under fluoroscopic guide 3 cannulated screws are passed into the femoral neck. This method is employed for young patients like the index patient. In addition,the fracture is well aligned. * For the femoral shaft fracture... Plates and screws are used. Intramedullary nails would not work for this fracture considering how the bone was severely fragmented. Prophylactic antibiotics and Wound drain is very important. He can be mobilized on crutches with little weight bearing after 2-3weeks, depending on the healing rate on x-ray film. Complications include avascular necrosis, infection, haemorrhage, nonunion... |
