@fodiyo, I suspect you have a condition called "anal fissure", though this is a suspicion. It could be an abscess (anal or around the anus). There may or may not be a hemorrhoid there as well. I think a change in diet might be in order, if you can stand it - that is eating meals that form low residue (mainly pap and soft boiled rice) for about a week. This will "appease" the constipation you experience, the primary cause of the "wound" I suspect you have in your anus. It will allow the wound there try to heal. If after 3-4 days, you will need to see a surgeon to deal with it decisively. @Aladunni avascular necrosis refers to "death of an organ or tissue" as a result of shutting off of its blood supply, in the absence of infection. It commonly occurs in people with blood anomalies e.g. sickcle cell disease, and people who are on steroids for a long time. AVN of the hips means that the bone at the point where the thigh and the hips meet is dead. For her to move, she'll need to have an artificial hip put in place. Hip replacement surgeries are being done successfully in several teaching hospitals like UCH Ibadan, UNN Nsukka, UCTH Calabar etc and on private basis by suitably qualified orthopedic surgeons. Like most cases in Nigeria, the relatively high failure rate is as a result of the poor compliance to the surgeons advise. Other times, its because of late presentation to the health facility, allowing the disease to advance to a "point of almost or no return". Pele. I'll recommend that she takes the surgery, and that she should make a hematologist (as well as an obstetrician in case she plans to have another child) her best friend. |

@naijamedic this is so good of you. God bless you Am so grateful for all that and i will make sure to get this information accross. We have actually made up our mind to go for the surgery but the hospital was a concern but since you ave given me what i need now, first thing is to go to ibadan which is the nearest to us and someone somewhere told us yesterday that UCH could be good. I also want to know how long it takes heal after the sugery? I really pray its successful, kindly reply ASAP thanks Thanks so much |

@U1, thanks men, I appreciate that. @Aladunni, thanks too, but I'm sorry to disappoint you that I'd prefer to keep our lines of communication here. Many people benefit from our discussions here and we must not loose sight of that fact. Regarding how long it will take for her to completely heal after surgery, so many factors are involved here e.g. her age, whatever medications she is has being taking or is currently taking, any other coexisting conditions etc. Her sickle cell disease state predisposes her to delayed wound healing and that as well as her age may contribute to also delaying the rate at which her bones will recover. One thing is sure, she will be bedridden for a long while and it is best she as well as her loved ones are prepared for that. Even after she has shown improvement, she will be "retrained" on how to walk by physiotherapists and this may be a long and difficult process. What is important is that her loved ones are around her and support her all the way!! Have a great day guys!! Naijaking? Balogun? KathyEkiti? When are you guys coming over?

@naijamedic no wahala my brother i get your point, no offence, afterall we are mostly faceless on this forum now this issue of being bedridden is a fear we have had, but since its unavoidable now, we must just face it. Thanks for taking your time out. I will report back after the ibadan trip or if there are more questions to be asked. Next question is for moi I noticed since last year October that my urine smells as if am using an antibiotic which i never used at that time or since then. The only time i used strong antibiotics was after a surgery I had mid last year. What is causing this? |

Abuzola: If you had a dental trauma, it becomes very important that you see a dentist ASAP. The major concern would be seeding very virulent organisms from your mouth into your blood. So you need antibiotics, pain medications, and appropriate oral hygiene. The dentist could decide on extraction upon exam. In addition to the pain, if you're already having fever/malaise, I would be very worried. |

Please doctor, My friend can not sleep peacefully, I have been leaving with this my friend for two years without any proble until few months ago he started complaining of dreaming every night and having headache almost everyday. He said he dream every night and most of the time act aggressively in the dream and I heard him twice talking while sleeping, trying to solve a problem. I heard him saying 'if the capital of the state is not in the book, then it is a new state'. Is it Sleep disorder? What can he do? |

Hey doc, I have had boils and anytime it's time to drain the pus (and after the usual screamings and shoutings) my body system just shuts down and I faint. I have to be revived back to consciousness. It has happened twice, though I have fainted thrice actually, but the third time was for something else. What do you think the problem is? |

Perhaps s/he is experiencing emotional suppression creating a state of mental or emotional strain or suspense due to his/her daytime stress making him suffer from fatigue and emotional tension resulting in his nightmares. You see, for one to understand the main cause and recommend a solution, one has to know more about the history, and the information you gave is not adequate to reveal that. However your friend might need to adjust and make a conforming manner of his/her activities with the mind and body balanced with the brain, being him a student or a worker I suppose s/he consults a psychotherapist for help. Meanwhile, he should be allowed to have enough time for sleeping because it actually takes some more time for the brain to sleep even though the body sleeps. |

Your temporal loss of consciousness "fainting" is due to a sudden decline in blood flow to your brain. It may be caused by an irregular cardiac rate or rhythm. Unconsciousness can also be as a reason of Diabetic Reactions, Anemia, Accidents, Heat Stroke, Cirrhosis of the liver, and many others. Please, see a Doc, things are not very right somewhere. |

It's not clear what exactly the problem might be because, i don't know the volume of your "boils"and "pus" neither do i know the part of your body where you're infected nor what infection they were. One was on the sole of my feet, the other. . . my armpit. . . and the pus volume was not something to go hysterical about Some other time I had a sore on my finger and it let out some pus, I quickly rushed to the bed because I suddenly felt dizzy. I have gone to the hospital a couple of times for the usual check up (not for this issue I have to add) The result's always good. |

stillwater: to shock. You go into shock sometimes as a result of trauma (bleeding can result in hypovolemic shock) or systemic infection/sepsis. Your body doesnt supply enough blood to the tissues and shuts down. since yours is just a simple boil its a mystery why you end up fainting . . . perhaps the doctor has an oppinion. |

stillwater: it may not have anything to do with volume of pus, if the pus gets into your blood stream it can trigger a massive systemic infection and lead to shock. |

4 Him:The frequent unconsciousness may not be as a result of the "boils" or "pus", but the pain and the prolong screaming (during "draining" can trigger a stress situation that could instantaneous affect her cardiac rythms. There are other considerable factors responsible for this development; i can't really tell of the state of her health whether serious or not, just giving instances on all possibilities. The main idea remains that she consults a Doc who will examine her properly Dear Stillwater, do not make the mistake of consulting traditional "healers", s/he'd most probably blame it on "Ogbanje" spirit |

Naijamedic, Naijaking and Kathyekiti, I am glad you are keeping this thread going. I have a few days with no admin work or teaching medical students. Should give me chance to contribute again to answering medical questions. For those of you in UK interested in doing part B and C of the MCEM exam, I am planning on writing a question and answer book for the exam. Adeturanyo. Your skin itch could easily be simple heat rash or parasites. Stop scratching to start with as you will end up opening skin and cause skin infection!! See your doctor regarding the itch, it could be scabies The problem under your armpit is most likely an infected sebaceous cyst |
