People who came here to give birth or just for treatment on medical tourist visa and then met and married US citizens went through adjustments and became citizens.

There also people who came to visit their family members and decided to stay and have become citizens also.

Most of the people I know first came as visitors/tourists before going on to become citizens.

Many programs are canceled and new ones introduced in the last 3 years that are different, but for most naturalized US citizens I know, it's been through tourist/visitor's visa.
