1. go and swear affidavit to that effect. though ur ctc will do but yr 2004 filled in ur form wil b questined since no proof. Just go wit the nikkai pix thats all.
2. U can tel dem u made a mistake while filling the form.
My advice is 1. Tel the truth cuz u wil swear that day.
